Patient already has a nephroureteral cather which was removed. Antegrade nephrostogram done. After this, two amplatz wires were advanced into the urinary bladder under fluoro.  Next a NephroMax dilatation system was used to dilate the tract and deliver a large caliber sheath into the stone bearing renal pelvis.  Another physician performed nephrolithotomy then a repeat an antegrade study  done. and a nephroureteral catheter was placed with the tip in the distal third of the ureter.  The catheter was sutured to flank and connected to gravity drain.  I think what's throwing me is it looks like they are exchanging a nehproureteral catheter for another nephroureteral catheter.  Is this 50387?
