failed fixation intra articular distal radius fracture

Patient had an ORIF(25609)  and at 1 month followup there is a loss of reduction and dislocation of the radiocarpal joint. Patient returns to the OR for removal of hardware and ORIF of distal radius fracture and ORIF distal radioulnar joint.  What cpt code would you use?
